EXTENSION REQUESTS MUST BE SUBMITTED TO TDA NO LATER THAN the applicable pest management zone's stalk destruction deadline or extended deadline.

ONLINE FORM: The online RCD-901E Extension Request for Cotton Stalk Destruction Date form has three types of requests to choose from:

Initial Request - An initial request must be completed only one time during the cotton season with your first field submission. Upon submission of the initial request, TDA will collect client and contact information and you will be provided a unique ID number, (Ex. CSD4). You must use this ID number for any additional field submissions.

Additional Field Submission - You have previously submitted an online initial request form and are requesting an extension on an additional cotton field.

Further Extension Date - You have previously requested an extension for the field and are requesting an extension to the previous requested date.

FAILURE TO PROVIDE COMPLETE INFORMATION MAY RESULT IN DELAY OR DENIAL OF REQUEST.
